  • CTABA’s PPLO Committee is gathering information from our members regarding issues with insurance carriers on authorizations, claims processing, and potential violations. If you have submitted a formal complaint to the Department of Insurance or the Office of the Healthcare Advocate, we are looking to compile and assess the complaints in order to better advocate for our members and providers in the future. Please complete the form separately for each complaint that you wish to log.  Please be mindful to exclude protected health information (PHI) from your report.
